As your group health plan, the Plan must use and disclose protected health information in order to facilitate benefit plans with our health care Providers. The “Plan” uses physical, electronic, and procedural safeguards to protect your personal information from being used or disclosed inappropriately. What is HIPAA?The Health Insurance Portability and Accountability Act of 1996 is federal regulation intended to restrict employers ability to use and disclose protected health information “known as PHI”.What do PHI (Personal Health Information) and HIPAA rules intended to protect?Information that relates to an individual’s medical condition, the provision of medical care for the individual, or the payment for that individual’s medical care. It also protects the health coverage or category, along with enrollment and premium payment information as well as information relating to a health condition and treatment. Information that identifies the individual to whom it relates and that is created or received by a covered entity of employer, with the exception of education records and employment records created or maintained by the employer.What is protected health information (PHI)?Protected health information is individually identifiable health information that is transmitted or maintained in writing, electronically, orally, or by any other means. It includes information created or received by the “Plan” that identifies a person and relates to the person’s participation in the plan, the person’s physical or mental health, the provision of health care services to that person, or the payment of health care services received by the person.Who has access to PHI?The following employees have access to PHI:
