Immunization Requirements

A. W. Brown Leadership Academy follows Texas Department of State Health Services Texas school and child-care facility immunization requirements as determined by the state legislature and set by the Texas Department of State Health Services, in conjunction with the Texas Education Agency in order for a student to attend school. The law requires that all students maintain adequate and up-to-date immunization records. The immunization vaccine requirements for each grade level are listed below on the A. W. Brown Leadership Academy website.

The immunization vaccine requirements for each grade level are available in the "Links" section.

Exemption Information - School Immunizations:

Texas Administrative Code (TAC) §97.62 describes the conditions under which individuals can seek exemptions from Texas immunization requirements. Exclusions from compliance are allowable on an individual basis for medical contraindications, active duty with the armed forces of the United States, and reasons of conscience, including a religious belief.

Requesting an Exemption for Reasons of Conscience:

To claim an exclusion for reasons of conscience, including a religious belief, the child's parent, legal guardian, or a student 18 years of age or older must present to the school or child-care facility a completed, signed and notarized affidavit on a form provided by the department stating that the child's parent, legal guardian, or the student declines vaccinations for reasons of conscience, including because of the person's religious beliefs. The affidavit will be valid for a two-year period from the date of notarization. **A child or student, who has not received the required immunizations for reasons of conscience, including religious beliefs, may be excluded from school in times of emergency or epidemic declared by the commissioner of the department.
*** If your scholar is enrolled in ImmTrac, the Texas Immunization Registry, you may use the Authorization to Release Official Immunization History Form to request official records of immunizations. You can find the link to the form under "Document Uploads" located to the left of the screen. ***
